To request that the school nurse or a trained staff member perform a healthcare procedure for your child, please complete the form linked below. The procedure will be carried out according to the physician’s instructions.

All equipment, supplies, or other items needed for the procedure must be provided by the parent or guardian. You are also responsible for maintaining and replacing these items as needed.
